Privacy policy

How we handle the information you share with us, written plainly rather than in legal shorthand. If anything here is unclear, please ask us.

Overview

This policy explains how LifeCare Assisted Living collects, uses, shares and protects personal information about residents, family members, visitors and website visitors. It covers our website at lifecareassisted.com and the care we deliver at 12512 Skyline Dr, Burnsville, Minnesota.

We keep two separate categories of information, because they are governed differently:

Care and health records

Information about a resident’s health, medications, care plan and daily life. Minnesota and federal law, including HIPAA, applies to this information and limits how we can use or share it.

Website and contact information

What you type into our enquiry form, and basic technical data such as your IP address and browser type. This policy governs that information.

Information we collect

You give us directly

  • Enquiry form: your name, phone number, email address and what you are asking about, when you complete the contact form on this site.
  • Email, text and phone: whatever you choose to tell us when you get in touch.
  • Tour enquiries: if you visit, the name and contact details you leave with us.
  • For residents: admission paperwork, medical history, medication lists, emergency contacts, your care plan and any belongings record.

Collected automatically

  • Basic technical data: your IP address, browser and device type, and the pages you visit. We use this to keep the site working and to spot abuse.
  • Cookies and similar technologies, described in more detail below.

How we use your information

  • To respond to your enquiry. If you submit the contact form, a member of our care team contacts you, usually within one business day.
  • To arrange a tour or answer questions about availability and cost.
  • To provide care, if you become a resident, and to keep your care plan accurate.
  • To send you our monthly article letter, if you asked for it. Every letter has a one-click unsubscribe.
  • To keep the site secure and to understand which pages are useful.
  • To advertise our homes and measure which adverts lead to enquiries, as described under Advertising and marketing.
  • To meet our legal and regulatory obligations.

We do not sell your personal information. We do not rent mailing lists. We do not share care or health information with advertisers, and that information never leaves the care team without your written permission, unless the law requires it.

When we share information

We share the minimum needed, and only with people who need it to do their job. That includes our own staff, your chosen doctors and pharmacy, and any home we move you to. Where a service provider acts on our behalf, such as our family app provider, we sign a written agreement obliging them to protect the information the same way we do.

  • With your consent, for example when you ask us to coordinate with a specialist.
  • With advertising platforms, limited to the measurement described under Advertising and marketing, and never with care or health information.
  • Between LifeCare homes, if you move from one of our homes to another.
  • Where the law requires it. We will tell you, unless we are legally barred from doing so.
  • In an emergency, where delay could put a resident at risk.
  • As part of a sale or reorganisation of the business, in which case this policy continues to apply.

Cookies and tracking

This site uses a small number of cookies. We keep the list short on purpose. When we run advertising campaigns, our advertising partners set additional cookies, described under Advertising and marketing below.

Advertising and marketing

We advertise our homes through platforms such as Google and Meta (Facebook and Instagram), and we may work with other advertising platforms in the same way. This section explains what those platforms receive when you visit our site, so the policy is already clear before and after any campaign runs.

What advertising platforms receive

  • Basic technical details: your IP address, browser and device type, and the pages you view here. Platforms collect this through cookies or a tag such as the Google advertising tag or the Meta pixel.
  • Standard events, such as a page view or the start of an enquiry, so we can tell which adverts turn into real conversations.
  • Nothing else. We never send health or care information, resident records, or anything you type into our forms to an advertising platform.

How they may use it

  • To measure how well our adverts work, in reports we act on in aggregate.
  • To limit how often you see the same advert, and to avoid showing you one you have already acted on.
  • To show our adverts again to people who have already visited our site, where the law allows it.
  • Platforms are contractually restricted to our own campaigns: they may not use this information for themselves or pass it on to anyone else.

We do not sell your personal information. Sharing with advertising platforms is limited to measuring and improving our own campaigns, never with care or health information, and you can opt out at any time using the choices below.

Your advertising choices

  • Opt out of personalised adverts from Google, and read how Google uses information from sites and apps that carry its tags.
  • Control adverts from Meta in the ad settings of your Facebook or Instagram account, and read the Meta privacy policy.
  • Use the industry-wide opt-out page to turn off personalised advertising from many companies at once.

Minnesota law also gives you the right to opt out of targeted advertising. You can use the choices above, or ask us to do it for you using the contact details at the end of this page. Where your browser sends a Global Privacy Control signal, we treat it as an opt-out request where that signal applies.

If the law that applies to you requires consent before advertising cookies are stored, our advertising tags wait for that consent.

How long we keep it

We keep information only as long as it serves a purpose or the law requires it.

  • Enquiry form details: deleted within 24 months unless you become a resident or ask us to keep you updated.
  • Resident care and health records: kept for at least six years after the last date of service, then destroyed, following Minnesota law and our record retention policy.
  • Business correspondence: kept for the period needed to show what we agreed to and when.

Your choices and rights

You can ask us at any time to see the personal information we hold about you, to correct it, or to delete it where we are not required to keep it. There is no charge for this.

  • Access and correction: ask us for a copy of your information, and tell us if something is wrong.
  • Marketing email: unsubscribe using the link in any letter. We stop sending it and keep no further record of you as a subscriber.
  • Targeted advertising: opt out using the choices listed under Advertising and marketing, or ask us to switch it off for you.
  • Resident records: a resident, or a legally authorised representative, may request the full record under HIPAA. We usually respond within 30 days.
  • Withdraw consent: where we rely on your permission, tell us and we will change how we use your information going forward.

If you are unhappy with how we have handled a privacy concern, you can contact us first, and you also have the right to file a complaint with the Minnesota Attorney General’s Office or the U.S. Department of Health and Human Services.

How we protect it

Care records are kept in locked files and on access-controlled systems. Only staff involved in your care can open them, and every access is recorded. Paper forms are shredded when they are no longer needed. Our website is served over encrypted connections.

  • Staff sign confidentiality commitments as a condition of employment.
  • Training on privacy and HIPAA happens at onboarding and is repeated each year.
  • Devices used to record care information are password protected and locked when unattended.

No system is perfect. If a breach affects your information, we will tell you and the relevant regulator without unreasonable delay, and explain what we are doing about it.

Children

This site is intended for adults and for families making care decisions. We do not knowingly collect information from anyone under 16. If you believe a child has given us information, contact us and we will delete it.
