A small home, built around one idea

Care should feel like home. LifeCare Assisted Living runs two eighteen-bed homes in Burnsville, Minnesota, with registered nurses on site, family staying close, and staff who have been here long enough to know every resident by name.

Why we stayed small

Most of our families arrive after a hard search through large communities. What they want is simple: someone who knows their mother's morning routine, a nurse who answers the phone, and a room she can move around in without asking.

Size is what makes that possible. With eighteen residents we can hire people who stay for a decade, notice a resident has stopped eating, and tell a family the same day instead of at the next shift change.

  • Registered nurses on site, not on call
  • Memory-care trained staff on every shift
  • Family app with live daily updates

Our vision

A community where growing older never means giving up home, dignity, or the people you love.

We imagine Burnsville families never having to choose between clinical skill and a place that still feels like a house on a quiet street.

Our mission

To run small, nurse-led homes where every resident is known by name, every family stays close, and care is delivered with skill and patience.

Never like an institution, always more like the home your family already knows, staffed by people you would trust with your own parents.

Four things we refuse
to compromise on

Not slogans on a wall, but the reasons families choose us, and the reasons we turn down growth that would break them.

  1. A home, not a facility

Two small houses with eighteen residents, so everyone still knows everyone. No long corridors, no institutional routines.

  2. Nurses on site

Registered nurses live in the home, not on call elsewhere. Medication, observations and care plans are handled by someone already there.

  3. Memory care that is calm

Structured days, familiar routines and dementia-care training for the staff, so good days outnumber hard ones.

  4. Families stay close

A live family app, open visiting and honest updates. You should never have to chase us to know how your loved one is doing.

Fourteen years,
one direction

We have grown slowly on purpose. Every step below made the homes better without making them bigger.

  1. 2011

    Two homes, one idea

LifeCare opened its first small house in Burnsville with eighteen beds and a simple rule: care should feel like living at home.

  2. 2016

    Memory care added

The second house opened after families asked for dedicated dementia support rather than folding it into general care.

  3. 2021

    Nursing brought in-house

We stopped outsourcing clinical cover and hired our own registered nurses, so care plans never sit with a stranger.

  4. Today

    Still eighteen residents

We have grown deliberately slowly. The homes are full, the staff have been here for years, and we have no plans to change that.

The people
you will actually meet

We hire for warmth first and credentials second, then train relentlessly in both. Hover a card to learn more about each lead.

What families
and staff say

Real voices from the people who live this work every day, and from the families who trust us with someone they love.

  • Every resident has a story, and I treat each one with the same love and respect I would give my own family.

    Tigist Fiyisa Lead Care Coordinator
  • They knew Mom by name on day one. The nurses call before I have to ask, and the house feels like a house, not a ward.

Marisol D. Daughter of a resident
  • My father had a hard time settling anywhere else. Here the days are steady, the staff stay, and he actually looks forward to activities.

  • Eighteen across two homes. Small enough that staff know every resident by name, their routines and their preferences.

  • Yes. We work with residents on Medical Assistance (BI/CADI) waivers and can walk your family through the process.

  • Registered nurses are on site around the clock. We stabilise first, call 911 when needed, and update the family before anything else.

  • Yes, with the exception of a short quiet period after lunch when residents usually rest. There are no set visiting hours.
